EEVB at PO Box 49, Hampton Bays, NY 11946.
Include child's name, age, clinic site and night.

Please note that online registrations without payments will not be considered complete. Either pay using the credit card option or send an email saying you have mailed a check. There is a $25 service fee added on all payments except the $50 deposit. To avoid the service fee send a check by mail.
