Include captain's name, division, league site, and league night.

Male:Female Ratio: 4s: Any combination of males and females is acceptable from all males to all females.

Fees: 

• Full Price: $440
• Early Bird: $410 if paid in full by May 18
• $250 FIRST 6 TEAMS PAID IN FULL
• Please note that if you pay with a credit card there is a $25 service added for each transaction. Avoid the credit card fee by mailing check.
